Artist report card—FOR COLLEGES ONLY

After the show, we ask that you take 90 seconds to fill out the APCA report card, located at: www.apca.com/reportform.html AND download the NACA report card and fax it in. It's essentially the same form sent to two different organizations. Thank you for your efforts!
